If your team has a Canadian player and do not have a USABH ID please have them use this waiver : 2023 US Outdoor Canada Waiver
If a player on your roster is a Canadian Player without a USABH ID please put CAN-00000 in the USABH ID Box on the form.

    Team leaders responsibility to check list before submitting roster not the tournament director.

    If a restricted player plays without prior approval the game will result in a forfeit.

    Tournament directors reserve the right to add and/or subtract players from this list as they see fit.

    Rink Information

    As a team leader I understand there are two locations(with multiple rinks) for this event and it is my responsibility to ensure my team is at the correct location/rink on time.
